29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer fails to inspection the job sites when the scope of work changes and excavations are introduced into areas where workers operated aerial lifts. This was most recently observed at MCAS Miramar, Hangar 7 where the employer failed to conduct inspections of the worksite that recognized the hazards of aerial lifts conducting work activity near uncovered excavations.
