29 CFR 1910.119(d)(2)(i): Process safety information pertaining to the technology of the process did not include the elements specified in 29 CFR 1910.119(d)(2)(i)(A) through (E): a)At the establishment, the compilation of process safety information for the ammonia refrigeration unit did not include information pertaining to the technology of the process including the safe upper and lower limits for such items as temperatures, pressures, flows, or levels. b)At the establishment, the compilation of process safety information for the ammonia refrigeration unit did not include information pertaining to the technology of the process including an evaluation of the consequence of deviations, including those affecting the safety and health of employees. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
