Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught-by the downward stroke of the Hydraulic Multipress, P8185: a/ At the Hydraulic Press Area, operator(s) were inadequately trained to recognize the hazard of how the stroke adjustment mechanism attached to the Hydraulic Multipress, P-8185, affected the safety control distance formula for the two-hand control guarding device. One operator received an amputation of the first digit of the right index finger on or about 03/10/10. One method of abatement to correct this apparent hazard is to perform adequate operator training as required by ANSI B11.2, paragraph 4.2.4.
