Text For Citation: 01 Item/Group: 001A Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and caught by hazards when working from an elevated work platform suspended by a rough terrain fork lift: (a)Construction site at 606 South Bush Brothers Drive, Augusta, WI; Employees were exposed to fall/ caught by hazards at elevations of approximately 15 feet, while working from a personnel lift platform that: 1) was not secured to the forklift backrest, and 2) had floor dimensions greater than the width of the truck (measured across the load bearing tires, plus 10 inches on either side). AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE MEANS OF ABATEMENT COULD INCLUDE, BUT MAY NOT BE LIMITED TO; 1) ENSURE THE PERSONNEL LIFT PLATFORM IS SECURELY ATTACHED TO THE FORKLIFT LIFTING CARRIAGE AND FORKS IN ACCORDANCE WITH THE ACCEPTABLE REQUIREMENTS OF THE AMERICAN NATIONAL STANDARD INSTITUTE/ INDUSTRIAL TRUCK STANDARDS DEVELOPMENT FOUNDATION (ANSI/ ITSDF), SAFETY STANDARD FOR ROUGH TERRAIN FORKLIFT TRUCKS, ANSI/ ITSDF B56.6-2005, PART II, SECTION 5.15.1(b), ELEVATING PERSONNEL, AND 2) ENSURE THE PERSONNEL LIFT PLATFORM IS DESIGNED AND CONSTRUCTED ACCORDING TO THE ACCEPTABLE REQUIREMENTS OF THE ANSI/ ITSDF SAFETY STANDARD FOR ROUGH TERRAIN FORKLIFT TRUCKS, ANSI/ ITSDF B56.6-2005, PART III, SECTION 8.24.1, PLATFORMS FOR ELEVATING PERSONNEL. Abatement certification and documentation are required for this item.
