29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazards in the workplace: Employees entered tank 242 under a confined space permit to perform maintenance and cleaning using full face chemical cartridge respirators. Although air monitoring was done during this work at the tank entry opening, no air sampling was done in the area where employees were actually working when they were assisting the removal of sludge and cleaning the tank floor. Employees were exposed to unknown levels of gasoline vapors which contains hazardous chemicals, including but not limited to, toluene, xylene, trimethylbenzene, hexane, ethyl benzene, naphthalene and other hydrocarbons.ene,
