Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to structural collapse of the trusses and walls: a)Southeast corner of structure at the garage; On or about 6/10/09 and at times prior thereto, the employer did not ensure that all top cord temporary lateral restraints (TCTLR) were installed as the roof trusses were erected. b)Southeast corner of structure at the garage; On or about 6/10/09 and at times prior thereto, the employer utilized strips of oriented strand board (OSB) in lieu of 2X4 stress graded lumber for top cord temporary lateral restraints (TCTLR). c)Southeast corner of structure at the garage; On or about 6/10/09 and at times prior thereto, the employer did not install top cord and/or web member diagonal bracing and bottom cord temporary lateral restraint (BCTLR) after erecting 5 trusses. d)South center of structure at main entry area; On or about 6/10/09 and at times prior thereto, the employer did not provide for initial exterior/interior ground bracing before and after installing the first two trusses. Note 1: Abatement certification IS required for this item. Note 2: A feasible means of abatement would be to utilize/follow the WTCA (Wood Truss Construction Association)/TPI (Truss Plate Institute) BCSI (Building Component Safety Information) publications, BCSI-B1: Guide for Handling, Installing, Restraining & Bracing of Trusses; and BSCI B-2: Truss Installation & Temporary Restraint/Bracing for all wood truss erection./Bracing
