Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Express Energy Services, at the worksite of Lariat Services Rig #14, Odessa, TX. For the period of time up to and including 05/24/07, an employee working from a height of approximately 50-feet above ground level was exposed to hazards in the following instances: a) The EZ Stop II Shockwave Double Lanyard with Energy Absorber used by the employee was not used properly in that the carabiners were connected directly to its own webbing. Feasible and acceptable abatement methods would include: Ensure all employees are trained in the use of all personal protective equipment they are required to use.
