Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being ejected from the forklift trucks during the operation or being crushed by the fork lift in the event of a roll over: a.Fedex Express, Madison, AL in the loading/unloading area; On or about 11/8/06 - the powered industrial truck (Yale #N496082) was operated without a restraint device. Among other methods, one feasible method is to comply with the requirements of the American Society of Mechanical Engineers document ASME B56.1-1993; Safety Standards for Low Lifts and High Lift Trucks; Section 7.39 Operator Restraint Devices.
