Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to a crushing hazard while operating a vibratory asphalt compactor, in that: (A)Job Site: On or about June 30, 3006 and at times prior thereto, employees operating an Ingersoll Rand, vibratory asphalt compactor, model DD-34HF, product number 171729, were not protected from crushing hazards while exposed to a vertical drop of approximately 16.8 feet since the roller did not have a rollover protective structure (ROPS) in place. (B)Job Site: On or about June 30, 2006 and at times prior thereto, employees were operating mobile equipment such as, but not limited to, compactors, blades and dump trucks adjacent to a wall approximately 16.8 feet in height without the aid of a warning system to alert them when they were approaching the edge. Note 1: Feasible and acceptable abatement methods to correct this hazard may include the following: (A)Installation of the ROPS on the compactor as required by the equipment manufacturer. (B)Installation or utilization of a warning system such as barricades, hand or mechanical signals or stop logs when operating mobile equipment adjacent to vertical walls or sloped edges to alert the equipment operators of their proximity to the hazard. Note 2: Abatement certification and supporting documentation are required for this item.
