Text For Citation: 01 Item/Group: 001A Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Multiple injuries and death resulting from an improperly used forklift personnel platform. a) At the site, employees were exposed to a suspended platform tip over and/or fall hazard from the platform that was over 8 feet wider then the wheel base of the rough terrain fork lift truck, was not attached to the mast or forks, and the workload and weight of the platform was not posted on the platform. The elevated personnel work platform was not designed, constructed, and/or used in accordance with the acceptable requirements of the American Safety Standards for Rough Terrain Forklift Trucks, ASME/ANSI B56.6-2002 and other acceptable industry practices. For example, the platform did not meet the following design and use requirements of ASME/ANSI B56.6a in the following areas; (1) Section 8.24.1(b) Platform dimension exceeded in width more than 10 inches greater than the overall width of the truck on either side, (2) Section 8.24(f) Platform did not have the maximum work load and empty weight information prominently marked on the platform, (3) Section 8.24(h) The platform was not secured to the lifting mechanism. Among other methods, feasible and acceptable means to correct these hazards are to use safer alternate methods such as a scaffold or use a platform approved by the manufacturer or meet the design, requirements of ASME/ANSI B56.6-2002 for all attachments on rough terrain forklifts. No Abatement Certification or Abatement Documentation required.
