Text For Citation: 01 Item/Group: 001A Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by the heavy steel sides of concrete forms: (a) For the employee(s) required to remove concrete forms on columns poured at Medical Building #4 of the Sioux Valley Hospital project at 1215 South Lake, Sioux Falls, SD. Among other methods, one feasible and acceptable method to correct this hazard is to develop and implement a written procedure and to train employees in the safe practices for disassembly and/or removal of concrete forms as identified in ANSI A10.9-1970, American National Standards Institute, Safety Requirements for Concrete Construction and Masonry Work, Section 6.4.5 which states, "Vertical forms being raised or removed in sections shall not be released until adequately braced or secured".
