Text For Citation: 01 Item/Group: 001A Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)American President Lines Ltd (Maintenance Shops): On or about January 28, 2009, employees were exposed to electrical shock hazards created by: (1) Electrical panels considered as being locked-out without the use of actual lockout devices and proper identification installed. (2) The program was not reviewed annually to identify compliance and any changes. (3) Locks used to lockout equipment and electrical panels were not identified specifically as lockout devices. One feasible means of abatement, among other methods, would be to utilize locking devices which are identified as to their purpose, the person working on the component, date of lockout and the expected requirements for completing work as required by American President Lines; in addition, to perform an annual review of the program and retrain employees as necessary for any changes to procedures.
