Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by the forklift and/or falling off the forklift. a)Molding Area, on or about 10/02/03: Employee was operating a Clark Fork Truck model number 500EC, without any seat belts. b)Molding Area, on or about 10/02/03: Employee was operating a Clark Fork Truck model number 500EC, without a battery restraint device approved by the manufacturer. ABATEMENT NOTES: Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. Install seat belts on the Clark Fork Truck as per retrofit kit from manufacturer. 2. Install battery restraint device on the Clark Fork Truck as per retrofit kit from the manufacturer. 3. Follow the requirements of American National Standards for Safety Standard for Low Lift and High Lift Trucks (ASME B56.1-1993, Chapter 7.38) Abatement certification must be submitted for this item.
