Text For Citation: 01 Item/Group: 001A Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of burns, smoke inhalation from fire. a. A fire occurred on PSM-2. A Code 99 alarm was sounded, employees at the North end of PSM 2 did not hear the alarm, some employees did not know where the closest exit to evacuate was, some employees were on the outside of the building while others were told to stay in the break area. b. A fire occurred in the supervisors break room, a General Evacuation Code was sounded, strobe lights went off in the Bulk to evaculate, the employees evacuation was hindered due to the truck traffic moving in the yard. Employees in the cafeteria, union representative offices, Administration and Foreign buildings were not aware of the evacuation as the alarm could not be heard in these areas. Among other methods one feasible and acceptable abatement method to correct this hazard is to provide employees effective training on the established "Fire Emergency Procedures" and ensure that the alarm system is working and is clearly audible.
