Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed by and/or caught in between the forklift and the concrete pavement: a) On or about 6/20/2001 at the SE 17th Street Causeway site, an employee was observed operating a Caterpillar forklift and was not wearing the installed safety restraint system. Among other feasible methods, acceptable methods of abatement to correct this hazard are: 1.The employer must require and ensure that operators wear the safety restraint systems when operating forklifts that are equipped with them. (References: ASME/ANSI B56.1-1993 Safety Standard for Low lift and High Lift Trucks) 2.Inform employees of the forklift's safety features such as but not limited to safety restraint systems; and how to correctly use them when operating the equipment.
