Text For Citation: 01 Item/Group: 001A Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to a fire and explosion hazard from not being adequately trained on appropriate procedures when cyclones developed hot spots. (b) Employees were exposed to a fire and explosion hazard from the snuff steam line being locked out and were not able to quickly activate the snuff steam to put out hot spots in the #4 cyclone. Among other methods, one feasible and acceptable method to correct these hazards would be to develop procedures to follow when cyclones develop hot spots and train personnel on these procedures.
