Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) jobsite, Wakefield YMCA- Employer did not ensure that steel lintel, W24X55 weighing 2860 pounds, was set on the pilaster with lateral support according to the construction plans. There is no indication of lateral bracing for the pilaster, P3. The likely cause of failure of the pilaster and wall system was the lack of lateral support above the base of the pilaster. The lintel was located 23 feet above the area employees were directed to work. This exposed employees to a hazard that resulted in a workplace fatality on December 18, 2000.
