29 CFR 1926.451(e)(9)(i): The employer did not provide a safe means of access for each employee erecting or dismantling a scaffold where the provision of safe access was feasible and did not create a greater hazard: a) Job site, 1462 Chateau Street, Pittsburgh, PA: On or about January 22, 2026 during scaffold erection activities, the employer did not ensure employees used a safe means of access to get on/off the scaffold in that they were climbing the scaffold end frames of a walk through scaffold.
