29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: At a worksite located at 2630 McClelland Boulevard, Joplin, MO 64804: Employees were exposed to asphyxiation and thermal burn hazards. This was demonstrated when employees entering a walk-in freezer where large quantities of dry ice were being stored were exposed to unsafe levels of carbon dioxide gas. The employers personal protective equipment (PPE) assessment did not assess for the hazard present or the the potential hazards associated with the use of dry-ice that necessitated PPE in the form of respiratory protection for specific working conditions.
