On or about 8/11/2025, procedures were not utilized for the control of potentially hazardous energies when an employee was engaged in set-up operation's on the HAAS, Mold Machine, Model VM-3, Serial Number 36545. b) Armstrong Mold Corporation, 6910 Manlius Center Road, East Syracuse, New York, 13057, Pattern Shop. On or about 8/13/2025, procedures were not utilized for the control of potentially hazardous energies when an employee was engaged in set-up operation's on the HAAS, Vertical Mill, Model VF 3D, Serial Number 16761.
