29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure the JLG Model: G6-42A forklift was marked for use with the approximately 15 foot long homemade boom lift attachment. This condition exposed employees to struck-by hazards. (b)Green Light Remodeling & Construction, LLC, at 133rd Street and 395th Avenue, Bath, SD 57427: On or about and at times prior to July 22, 2025, the employer did not ensure the JLG Model: G6-42A forklift was marked for use with the approximately 30 foot long homemade splitter attachment. This condition exposed employees to struck-by hazards.
