29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: On or about May 6, 2025, employees were exposed to fall hazards of approximately 9 feet while welding tubs on the Mixed Model Line without the use of fall protection. MIDWEST TRAILER MANUFACTURING LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1910.28(b)(1)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1515664, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON 07/16/2021, WITH RESPECT TO A WORKPLACE LOCATED AT 2000 KENTVILLE RD., KEWANEE, IL 61443.
