29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about April 29, 2025, on the northeast side of Tank A, employees were not provided work instructions, information on safety standards, and training on avoidance of unsafe conditions specific to the rigging, landing, securing and visual inspection for jumping a climbing formwork into place under low light conditions.
