29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: 43 Northampton Ave. Springfield, MA: On or about March 3, 2025, two employees were exposed to fall hazards of 21 feet 5 inches while performing a residential construction on a rooftop while wearing fall protection harnesses without the lanyards being attached to the wearer's back.
