29 CFR 1910.29(b)(2)(i): Mid rails were not installed at a height midway between the top edge of the guiderail system and the walking-working surface: a) On or about March 13, 2025, employees were not protected from falling more than 4 feet to the concrete floor in that the employer did not ensure that the guardrail system of an expanded metal working platform in the mixing area of line 1 had mid rails installed on it.
