29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems that were used: Site: 133-25 37 AVENUE, Flushing, NY 11354. a). On or about March 03, 2025, and at times prior thereto, during facade/balcony dividers installation on a 16-story building, BCG Glass Industry Inc. failed to assure employees using personnel fall protection systems such as K-Strong UAF30090, Werner A510000 anchors, and Palmer Safety 8 feet Self-Retractable Lanyard were trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting of these systems. The employees were working on exterior balconies that were not guarded and exposed to fall hazards of up to 100 feet.
