29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): On or about September 10, 2024, the employer did not assess the workplace to determine PPE requirements for employees in the milling area of the facility located at 1 High Tech Avenue, PAINESVILLE, OH 44077. The result of no hazard assessment led to partial finger amputation of an employee that was operating the Bridgeport milling machine with gloves on.
