29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about 2/7/2025, the employer did not develop and/or enforce the use of machine specific procedures or additional methods and instructions for the control of hazardous energy sources such as but not limited to electricity or pneumatic energy. Employees tasked with servicing/maintaining the following equipment were not protected from caught-in or struck-by hazards from unexpected energizing, startup or release of stored energy: a) Axxis Router (Equipment ID# 1099, Located on Pre-Side between C10 and C11) b) Sawteq B-300 (Equipment ID# 1568, Located on Pre-side between E12 and F12) c) Stile Press #3 (Equipment ID #1114, Located on Pre-Side at F10) d) Martin T20 Shaper (Equipment ID # 1105, Located on Pre-Side between E11 and F11) e) Unique 4500 Shape and Sand Machine (Equipment ID # 1594, Located on Pre-Side, West E10)
