CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: 194 Cleveland St. Greenfield, MA: On or about January 24, 2025, and at times prior, employees were exposed to fall hazards when they used a 16 feet 6 inch tall ladder that did not extend 42 inches above a second floor landing surface.
