29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation. At 6025 Byasse, Hazelwood, MO, 63042 on January 22, 2025 and at times prior thereto, employees were exposed to struck by and caught between hazards due to an employee not completing training and evaluation before the operation of a powered industrial vehicle. On December 30, 2024 an employee was allowed to operate a Hyster 60 powered industrial vehicle s/n FK24423Y without the necessary training and evaluation.
