1910.178(a)(4) Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval. On or about January 7, 2025, and at times prior, south of County Road EW 1360 at State Highway 56, employees were exposed to crushed-by hazards while operating an open-cab multipurpose off-highway utility vehicle (MOHUV) on and off the roadway.
