29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe and defective rigging equipment was not removed from service: On or about December 17, 2024, at the bridge, employees utilized damaged rigging equipment to lift girders for a bridge support without inspecting the equipment prior to use.
