29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process: a)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation at 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not ensure the Product Realization Operating Procedure Manufacture of PY-951 Document Number 710054 Revision N, provided clear instruction for air drying PY-951, allowing evaporation of methyl ethyl ketone, prior to screening as described in Section 5.4.11. Screening PY-951 with a higher or lower than ideal concentration of methyl ethyl ketone can increase risk of detonation. The operating procedure does not provide clear instructions for operators to identify an appropriate endpoint for air drying of PY-951. This condition exposed employees to explosion hazards. b)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation at 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not develop and implement the Machine Operating Procedure Sweco Vibratory Screener Document Number 600086-050 Revision F including clear instruction for operators addressing the upset condition of a blinded screen as described on page 3 of 20. An employee suffered burns following detonation of PY-951 when addressing the upset condition of a blinded screen. This condition exposed employees to explosion hazards.
