29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a. Production Area: On or about November 29, 2024, and at times prior, the employer exposed employees to entrapment and caught-in hazards in that the employer allowed employees to enter confined spaces in four mixers and two pits without first evaluating the spaces to determine if they were a permit required confined space.
