29 CFR 1910.119(d)(2)(i)(E): The employer failed to perform an evaluation of the consequences of deviations, including those effecting the safety and health of employees: (a) Throughout the facility; On or about November 20, 2024, a compilation of written process safety information to include initial and previous air monitoring results for heavy metals including, but not limited to, lead and cadmium was not utilized to evaluate the consequences of deviations that would affect employees' health.
