29 CFR 1910.28(b)(11)(i): The employer did not ensure that each employee exposed to an unprotected side or edge of a stairway landing that is 4 feet (1.2 m) or more above a lower level is protected by a guardrail or stair rail system: On or about November 19, 2024, an employee was exposed to a fall hazard of approximately 5.5 feet while operating the trial size machine because stairway opening was not guarded.
