29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Midsun Group, Inc. at the jobsite located at 2385 Campbell Road in Sidney, Ohio; Instance B On or about November 14, 2024, the employer did not ensure each employee was trained in the recognition and avoidance of unsafe workplace conditions, and the applicable regulations to control or eliminate hazards. Employees engaged in the refurbishment of the middle metal clad switch gear cabinet were exposed to falls of approximately eight feet in that the fall arrest systems worn by employees were rigged in a manner that would not prevent a free fall of more than six feet, nor contact any lower level.
