29 CFR 1910.269(a)(3)(i)(A): Contract employer(s) were not informed by the host employer of the characteristics of the host employer's installation that are related to the safety of the work to be performed, as listed in paragraphs (a)(4)(i) through (a)(4)(v) of this section; a) Location-320 West Main St., Sayreville, NJ 08872- JCP&L Substation On or about 5/24/2024, contractor employee(s) conducted air quality testing within a JCP+L sub-station using hand-held multi-gas meter(s) and air sampling pumps, such as but not limited to, a Lighthouse 3016-iaq Particulate meter. Electrical lines and/or conductors housed within the sub-station were energized during air quality testing work by employee(s). The contract employer was not informed of the energized state and voltage(s) of lines and equipment housed in the building in which contractor work was to be performed, exposing contractor employee(s) to electrical hazard(s). An arc flash incident occurred on 5/24/2024, in which employee(s) sustained burns to both hands and face.
