29 CFR 1910.23(b)(2): The employer did not ensure that ladder rungs, steps, and cleats were spaced not less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, or steps: On or about October 22, 2024, and at times prior thereto, in the Plant Two Production Area; catwalk guardrail rail systems being utilized as fixed ladders with rails spaced greater than 14 inches between centerlines were used as ladder rungs by employees conducting modular home decking and roofing operations in the following instances (1-4): 1) Station 15 (Decking Station). 2) Station 16 (Decking/Roofing Station). 3) Station 17 (Roofing Station). 4) Station 18 (Finish Station).
