29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when: The operator has been involved in an accident or near-miss. On October 11, 2024, the Buckeye Paper Co., Inc., failed to ensure retraining for an employee that was in a powered industrial vehicle accident prior to permitting them to resume work on the forklift. The employee operated the forklift for approximately 2 weeks until the day of their retraining.
