29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or about October 10, 2024, in ARU #6/7 at Deer Park Refinery Limited located in Deer Park, TX, the employer, DPRL, did not implement its safe work practices including but not limited to: a) Control over entrance into the process units ARU#6/7 when the contract employees did not sign in on the unit entry log before entering the process unit and they did not sign out upon leaving. d) The host employer (DPRL) did not complete the written contingency plan for the work permit# 411125.
