On a scaffold system, at a temporary worksite located, at or near, the northside of 1090 Bella Vista Boulevard in Saint Johns, Florida 32084: On or about September 10, 2024, the employer exposed employees to fall hazards ranging from 10 to 15-foot, in that the working level of scaffold was not fully planked or decked as employees performed masonry work. b.
