29 CFR 1910.333(a)(1): Live parts to which an employee would be exposed were not denergized before the employee worked on or near them, unless the employer could demonstrate that denergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations: 29 CFR 1910.333(b)(2)(ii)(B):The circuits and equipment to be worked on were not disconnected from all electric energy sources: a. At the mixing plant, on or about September 4, 2024, the employer exposed employees to a 120-volt electrical hazard, in that, the employer did not ensure that all electric energy sources were disconnected during maintenance and replacement of a circuit disconnect cabinet for the IMP screw machine.
