29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 08/13/2024, throughout the establishment, Lockout/Tagout (LOTO) procedures were not utilized when maintenance employees performed maintenance and servicing activities on equipment such as, but not limited to: exhaust fans and compressors, exposing employees to the unexpected startup of the equipment. b) On or about 08/13/2024, throughout the establishment, Lockout/Tagout (LOTO) procedures were not developed, documented, and utilized when maintenance employees performed maintenance and servicing activities on equipment with multiple energy sources, such as, but not limited to: air handling units, water pumps, and boilers, exposing employees to the unexpected startup of the equipment.
