Text For Citation: 01 Item/Group: 001A Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. On or about August 5th, 2024, employees engaged in commercial baking activities in the Roll-in production area for the facility located at 4657 G Street, Omaha, Nebraska 68117. The employer is failing to protect employees from flash fire hazards in the Roll-In Room in that flour dust is being suspended in the air during cleaning operations as a result of employees using compressed air to blow built up flour from the surfaces of their production equipment without de-energizing all potential ignition sources present in the area. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS INCLUDE BUT ARE NOT LIMITED TO: 1) Develop and implement a written procedure addressing the requirements of NFPA 61, 8.4.2.6.1 with the intent of ensuring equipment has been powered down and isolated prior to using the compressed air as means of dislodging built up flour dust. 2) Develop and implement a written housekeeping program pursuant to IAW NFPA 61 Section 8.4.2.1 with the intent of mitigating and/or eliminating combustible fire/explosion hazardous conditions. 3) Conduct a Dust Hazard Analysis (DHA) per NFPA 61 Section 7.1.2 to evaluate the flour dust fire and combustibility hazard within the Roll-In Room and verify what necessary measures need to be taken to prevent potential fire and explosion hazards and apply that information in the development of a housekeeping program. 4) Purchase additional equipment such as an explosion rated dust collector and explosion rated vacuums to as means of reducing excessive product accumulation. 5) Ensure employees are trained on the written programs and procedures listed in Items #1 and #2 above.
