29 CFR 1910.180(h)(3)(i)(b):The employer did not assure that the load was well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches. On or about July 18, 2024, at the Shooter McGavin South #2621 LS in Big Spring, Texas, an employee was near a counterweight that had been properly placed on the counterweight stack with a crane. As the crane was lifting the rigging to clear the counterweight, a portion of the rigging swung and caught the lifting ear of the counterweight. The slight lift caused the side of the counterweight to lift slightly and slide off the stack onto the employee.
