29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about June 24, 2024, at the 3rd floor West Wing Community Room, an employee pulled fish taped electrical wires through a conduit when standing from a CrossXStep ladder without the employer providing instruction on safe work position for fish tape.
