29 CFR 1926.20(b)(2): Accident Prevention Responsibilities. Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about May 16, 2024, the employer (PCC Masonry, Inc.) did not ensure that they had in place a program that included adequate frequent and regular inspections were being conducted by a competent persons designated by PCC for the large area scaffold job site located at 600 Buckskin Trail Xenia, OH 45385, in that PCC employee(s) were exposed to fall hazards due to improperly installed guardrails on the 36'- 6 buck scaffold, portions of the scaffold was not fully planked, a damaged fixed ladder that was bent, and the scaffold was erected over 24' and was not tied into the building.
