29 CFR 1910.23(b)(2): The employer did not ensure that ladder rungs, steps, and cleats were spaced not less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, or steps. a) Ammo International, Inc.- Throughout the warehouse/storage area: Employees were exposed to crushing injuries and falls from heights while using handmade ladders with rungs unevenly spaced, not leveled, and spaced approximately 5 to 8 inches apart, occurred on or about 5/15/2024.
