29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: (a) Customer Locations: On or about May 9, 2024, employees grinding metal welds were exposed to eye and face injuries from hot sparks and high-speed, metal particles. (b) Customer Locations: On or about May 9, 2024, an employee welding without gloves while wearing a short sleeve t-shirt was exposed to burn injuries.
